| 1 | Note time and determine: 1. Caller's name/phone number or location; 2. What happened, when and where?; 3. Are there any injuries?; 4. Who is/are injured?; 5. Extent of injuries?; 6. Is there a death? |
| 2 | All work associated with an accident which resulted in injury or death should be suspended. |
| 3 | Determine if scene is secure (i.e. scene is not dangerous to anyone rendering assistance.) |
| 4 | Direct/initiate first aid as appropriate. |
| 5 | If immediate medical attention is required, dial 9-911, (for the GBT warehouse outside line phone, just dial 911). Provide information as the situation dictates. |
| 6 | Notify NRAO Emergency Response Team (ERT) (if High Angle Rescue required). Reference ERT Roster. |
| 7 | Monitor situation and transmit additional information to 911 if required. |
| 8 | Inform safety officer, Johnny Samples at 2215 or home number. |
| 9 | Inform Mike Holstine at 2231 or home number. |
| 10 | Inform division heads as required. |
| 11 | Coordinate ambulance/ERT actions upon their arrival unless otherwise directed. |
| 12 | Update OpsLog of actions. |
